Medical negligence; who is to blame?

According to the World Health Organization (WHO), health care errors affect one out of every ten patients worldwide. So can Doctors and Nurses be blamed for mistakes that kill or seriously harm patients each year in our hospitals?
Medical mistakes result in part from complexities in health care systems, which create confusion, uncertainty and ineffectiveness. Every year a huge number of people in Ghana die unnecessarily or have their health significantly maimed as a result of medical malpractices.
